  • 8, 13 or 26-week renewable assignments
  • Hours:
    • Weekdays with multiple patient visits per day
    • Weekend shifts available
  • Home Health visits under the guidance of a nurse case manager
  • Travel radius is territory specific

Description of Work

Perform personal care activities that assist the patient with activities of daily living:

  • Personal Hygiene
  • Ambulation/Transfers
  • Dressing/Shaving
  • Prepares meals and snacks according to instructions
  • Accompany patient on errands or medical appointments
  • Performs light housekeeping activities
  • Providing hospice supportive care
  • Medication reminders
  • Assisting with the prescribed range of motion exercises
  • Assisting with blood glucose monitoring
  • Care of bed-bound patients
  • Incontinence care
  • Use lifting equipment such as gait belts and Hoyer lifts
  • Collaborate with physicians and other professionals to plan care
  • Make frequent visits to the patient to evaluate their condition
  • Provide support and comfort according to individual patient’s needs
  • Work with caregivers to plan and execute excellent patient care
  • Help in administering medication
  • Alleviate sudden symptoms of the disease’s end phase
  • Notify a doctor for a patient’s condition when deemed necessary
  • Assist patients and their families with paperwork
  • Maintain accurate records
  • Help relatives cope with the upcoming loss and provide post-mortem support
  • Knowledge of nursing care processes and medication guidelines/side-effects


  • Experience with OASIS Assessment tool
  • Minimum 1-year recent experience in a home healthcare setting
  • Current/Clear NC Nurse Licensure
  • Current/Clear BLS (American Heart Association)
  • Current/Clear vehicle insurance (must provide written proof)
  • Must have own reliable transportation
  • Great communication and listening skills
  • Committed to safety precautions and high ethical standards
  • Must be willing to visit clients throughout assigned territory
  • Combination of sitting, standing, bending, reaching, stretching, stooping, walking, climbing stairs and moving intermittently during working hours
  • Ability to lift/pull a minimum of 25 pounds
  • Successful completion of competency testing, drug screen, background check, physical exam, immunizations, and other agency screening/application requirements
  • Successful completion of other Client screening/application requirements
