Job Summary

Nationalhcs is seeking to fill Registered Nurse (RN) positions with home health experience in Raleigh, NC. The RN will coordinate and direct the home care patient’s care based on individual patient needs.


  • 13 or 26-week Renewable Assignments
  • Hours:
    • Weekdays with multiple patient visits per day
    • Weekend as well but not required
  • Home Health visits under the guidance of a nurse case manager
  • Travel radius is territory specific

Description of Work

  • Assess physical, functional, psychosocial, and cognitive status of home care patient utilizing interview observations and physical exam techniques through all phases of care taking
  • Assess the home environment for safety, infection control, and community resource needs
  • Collect information for assessment with the patient, family, physician, and other healthcare team members
  • Provide ongoing nursing assessment of the patient
  • Assess for the presence of advanced directives and facilitates further action in obtaining information about or implementing advanced directives, in needed
  • Provide treatment to patient per protocol, utilize OASIS assessment and documentation
  • Assist patient with daily activities (e.g., bathing, feeding, dressing)
  • Administer medication to patients
  • Work with other professionals to create treatment plans
  • Clean and redress wounds, if needed
  • Measure vital signs
  • Monitor changes in behavior and progress
  • Maintain a complete record of care given to the patient
  • Document in the patient medical record per home health agency policy and procedures


  • Minimum 1-year recent experience in home healthcare
  • Active/Clear Nurse Licensure valid in North Carolina
  • Current BLS (American Heart Association)
  • Oasis data experience
  • Direct care experience within the previous year in the US
  • Successful completion of competency testing, drug screen, background check, physical exam, immunizations, and other agency screening/application requirements
  • Successful completion of other Client/facility screening/application requirements
